LIBRARY BORROWING POLICY
The Canberra/Queanbeyan ADD Support Group Inc (ADDACT)
maintains its library for the information, use and benefit of its members.
These rules are designed to ensure that the greatest number of members can
have access to the resources in the ADDACT library.
Before materials are borrowed, these rules must be read, agreed to and
agreement on the next page signed by anyone wishing to use the ADDACT library.
LIBRARY POLICY
1. People wishing to borrow or otherwise consult material held in the ADDACT library must read and agree to these rules. Signing these
rules will constitute an agreement between the ADDACT and the
person who signed this form.
2. This agreement will become active only after it has been signed by a
person who has been formally authorised by the Committee of the ADDACT , to sign this agreement on behalf of the Group.
3. These rules apply to all materials in the ADDACT library.
'Materials" includes books, audio and videotapes.
4. Materials may be lent only to financial members of the ADDACT.
5. Any person borrowing material must provide their membership card at
the time of borrowing the material and also provide at the time of borrowing
their current contact details.
6. All material that is available for loan, is lent for a period not
normally exceeding 30 days, but may be returned sooner. If you wish to
extend the library book you must ring the SHOUT co-ordinator on 6290 1984
Monday to Friday.
7. No member may borrow more than one item at the one time.
8. It is the responsibility of the borrower to collect and return to the
ADDACT Group's library all materials lent.
9. Any material not returned to the ADDACT Group's library after 45
days will be invoiced at the replacement cost, plus an irrevocable
administration fee of $25.00, or where replacement is not possible, at the
original value of the item or $100.00, whichever is the greater.
10. The safety, good order and security of any material borrowed from the
library is the responsibility of the person who borrows the materials.
11. Materials may not be loaned or otherwise passed from the custody of
the person to whom the material was originally given. The original borrower
will remain responsible for their material until it is returned to the
library.
12. It is the responsible of the borrower into whose custody material has
been passed to ensure that the duty librarian of the ADDACT Group who
manage the library realise that the material has been returned in accordance
with these rules and amend the Library's borrowing list.
13. A person who breaches these rules may be excluded from borrowing
materials from the library.
14. The material in the library of the ADDACT is for the
information of those people who consult it. ADDACT does
not warrant the effective-ness, validity or appropriateness of any
information in the material and accepts no responsibility for any program,
action or activity based upon information that is derived from material held
in the Group's library. Responsibility for the use to which material is put
and any outcomes that arise rests with the person who uses the material.
